
Insurance
Alaska Chiropractic Clinic will submit your primary insurance at no cost to you. Please bring in a completed, original insurance form with the proper signature. Patients will be responsible for any additional insurance forms they wish to file. Deductible plus percentages not covered by the insurance company must be paid on a weekly or semi-monthly basis.
Patients are ultimately responsible for their bill. Insurance companies' contracts are with the patients, not the doctor. If the insurance company fails to pay, then payment is expected from the patient.
Cash Payment
We realize that payment on each visit may be inconvenient, therefore we have developed the policy of encouraging our patients to pay weekly or semi-monthly. We do require that each bill be paid within 14 days of receipt of statement. The clinic sends out statements the first week of every month. There are two copies of the statement forwarded to you. This enables you to file your own insurance should you desire to do so.
WORKERS' COMPENSATION
All workers' compensation cases must be verified by the employer or insurance carrier. Patients accepted on workers' compensation will be allowed to continue treatment without payment. If the claim is denied or controverted the patient is responsible for payment of the account.
PERSONAL INJURY
If you have had an auto accident, Alaska Chiropractic Clinic will bill the appropriate insurance company after the company's name, address, policy number and claim number are supplied. All assignment forms must be signed prior to accepting of patient's case. If all or part of the services provided are denied by your insurance company or third party liability, you will be fully responsible for payment of the unpaid balance.
MEDICAID
We accept Medicaid. However, Medicaid usually will not cover all services. You are responsible for services not covered. You must provide us with a Medicaid coupon as each month's services are rendered.
MEDICARE
We accept Medicare. However, Medicare will only cover chiropractic adjustments. X-rays, exams, physical therapy, ultrasound, electrical therapy, vitamins, minerals, massage, etc. are not covered. You are financially responsible for those services not covered. Medicare requires x-rays within the last year of any chiropractic adjustment even though they will not pay for the x-rays.
MONTHLY PAYMENT PLAN
For your convenience, we offer a monthly payment plan. A suitable payment schedule can be arranged with the bookkeeper, provided you have a good credit standing.

It is our policy that all supplies (vitamins, cold packs, support belts, etc.) are paid for when received. Thank you.

MILITARY
We offer a 15% discount for those in the military. A monthly payment plan is available for your convenience.
PAST DUE ACCOUNTS
If your account becomes past due and there is not a valid reason for delay in payment, appropriate action will be taken. If there are legitimate problems please discuss them wtih us so that we can work together to find a solution
